Semaglutide Dosage: What the Label and Trials Show
Semaglutide · Ozempic / Wegovy
Status & framing. Semaglutide is FDA-approved as Ozempic (type 2 diabetes) and Wegovy (chronic weight management) in branded pens. Research/compounded vials are not the approved products. Figures below cite labeled titration and trial context, not a recommendation to use non-approved products.
What the research shows
Approved pen products use once-weekly subcutaneous titration. Wegovy starts at 0.25 mg weekly for 4 weeks, then steps up toward a 2.4 mg maintenance dose when tolerated. Ozempic uses a related weekly titration path for glycemic control. Human evidence is extensive for the approved products; research vials are a different supply channel.
Doses used in studies
Each row is a dose as reported in the cited source, not a recommendation. Superscripts link to sources below.
| Context | Dose studied | Route / frequency |
|---|---|---|
| Wegovy label, titration start | 0.25 mg once weekly for 4 weeks[1]⚠ Approved product dose path. Compounded/research vials are not Wegovy. | Subcutaneous (approved pen) |
| Wegovy label, maintenance target | 2.4 mg once weekly (when tolerated)[1] | Subcutaneous (approved pen) |
| STEP / obesity trial context | Weekly titration toward 2.4 mg[2]⚠ Trial product and medical supervision context. | Subcutaneous |
Reconstitution & example math
A 5 mg research vial reconstituted with 2 mL bacteriostatic water yields 2.5 mg/mL. A 0.25 mg (250 mcg) example draw is 0.1 mL, or 10 units on a U-100 syringe. That is an arithmetic example only, not a protocol.
Approved products are pens with fixed dosing. Research vials require independent concentration math and cold-chain handling. Do not treat vial math as a substitute for labeled therapy.

Safety & side-effect notes from the literature
- Approved labels carry GI adverse effects, boxed/warning language around thyroid C-cell tumor risk in rodents, and multiple contraindications. Read the current label.[1]
- Compounded or research-channel products are not the FDA-approved pens and may differ in salt form, impurities, and dosing accuracy.[3]
