Survodutide (BI 456906) is a once-weekly peptide that turns on both the glucagon receptor (GCGR) and the glucagon-like peptide-1 receptor (GLP-1R). Zealand Pharma designed it and licensed it to Boehringer Ingelheim. It is not semaglutide, not tirzepatide, and not an FDA-approved drug.

TL;DR

  • Glucagon backbone, C18 fatty-acid chain. That chemistry is why a gut hormone analog can be injected once a week (Zimmermann et al., 2022).
  • Two jobs. The GLP-1 arm cuts intake. The glucagon arm raises energy expenditure and talks to the liver.
  • Phase 2 obesity: mean weight change at 46 weeks was -14.9% at 4.8 mg versus -2.8% on placebo (le Roux et al., 2024).
  • Phase 2 MASH: histologic MASH improvement with no fibrosis worsening hit 62% at 4.8 mg versus 14% on placebo (Sanyal et al., 2024).
  • SYNCHRONIZE-1 topline (company, not a journal paper): up to 16.6% mean loss at 76 weeks on the efficacy estimand versus 3.2% placebo. Survodutide is still investigational.

What is survodutide, and how is it different from semaglutide?

Most weight-loss peptide talk still starts at one receptor. Semaglutide is a GLP-1 agonist, tirzepatide adds GIP, and survodutide adds glucagon.

It is a 29-residue, acylated analog built from the glucagon sequence, not from GLP-1. A C18 fatty di-acid lets albumin carry it long enough for weekly dosing (Zimmermann et al., 2022).

Oxyntomodulin is the natural precedent. That gut peptide already hits both receptors. Survodutide is the engineered, once-weekly version of that idea.

Semaglutide versus tirzepatide is a separate comparison. The triple-agonist story is retatrutide. For the chemistry primer, see Peptides 101.

Why add glucagon agonism to a GLP-1 peptide?

GLP-1 receptor agonism slows gastric emptying, raises satiety, and lowers calorie intake. That is the familiar incretin half of the drug.

Glucagon receptor agonism is the other half. In mice, BI 456906 lowered body weight more than a maximally effective dose of semaglutide, from higher energy expenditure plus lower food intake (Zimmermann et al., 2022).

The liver is the glucagon story clinicians care about. GCGR engagement showed up as changes in hepatic enzyme expression, amino acids, and FGF-21, which is why MASH is part of the program.

Glucagon can raise glucose. Pairing it with GLP-1 is the design bet: keep the metabolic burn without losing glycemic control.

What did the phase 2 obesity trial actually measure?

le Roux and colleagues ran a 46-week dose-finding study in 43 centres across 12 countries. Adults were 18 to 75, BMI at least 27, and without diabetes.

Study Who was enrolled Design Main finding
le Roux 2024, obesity 387 adults, BMI ≥27, no diabetes Weekly 0.6, 2.4, 3.6, or 4.8 mg vs placebo, 46 weeks Mean weight -14.9% at 4.8 mg vs -2.8% placebo
Sanyal 2024, MASH 293 adults, biopsy-confirmed MASH, fibrosis F1 to F3 Weekly 2.4, 4.8, or 6.0 mg vs placebo, 48 weeks MASH improved, fibrosis not worse: 62% at 4.8 mg vs 14% placebo
SYNCHRONIZE-1 2026, obesity (topline) 725 adults with obesity or overweight, no type 2 diabetes Weekly 3.6 or 6.0 mg vs placebo, 76 weeks Company: up to -16.6% vs -3.2% placebo (efficacy estimand)

Planned-treatment analysis is the conservative number: -6.2%, -12.5%, -13.2%, and -14.9% across the four survodutide doses, versus -2.8% on placebo (le Roux et al., 2024).

Completion was a real limitation. Only 233 of 386 treated participants (60.4%) finished 46 weeks, and gastrointestinal events were common: 75% on survodutide versus 42% on placebo.

What did the MASH biopsy trial show?

MASH is metabolic dysfunction-associated steatohepatitis: fat, inflammation, and ballooning that can scar the liver. Dual GCGR/GLP-1R agonism is supposed to hit hepatic fat more directly than GLP-1 alone.

Sanyal and colleagues randomized 293 adults with biopsy-confirmed MASH and fibrosis stages F1 through F3 to 48 weeks of treatment (Sanyal et al., 2024).

Primary endpoint: histologic MASH improvement with no worsening of fibrosis.

  • 2.4 mg: 47%
  • 4.8 mg: 62%
  • 6.0 mg: 43%
  • Placebo: 14%

At least 30% less liver fat on imaging occurred in 57% to 67% on survodutide versus 14% on placebo. Fibrosis improved by at least one stage in 34% to 36% versus 22% on placebo.

Gut effects were not mild on paper. Nausea was 66% versus 23%, diarrhea 49% versus 23%, and vomiting 41% versus 4%, while serious adverse events were similar (8% versus 7%).

FDA later granted Breakthrough Therapy designation for non-cirrhotic MASH with moderate or advanced fibrosis. The phase 3 liver trials, LIVERAGE and LIVERAGE-Cirrhosis, sit outside obesity SYNCHRONIZE.

What did SYNCHRONIZE-1 report, and what is still unknown?

SYNCHRONIZE-1 is the first large obesity phase 3 readout (NCT06066515). It enrolled 725 adults with obesity or overweight without type 2 diabetes, on 3.6 mg, 6.0 mg, or placebo weekly for 76 weeks.

On April 28, 2026, Boehringer posted topline results, not a peer-reviewed paper. Using the efficacy estimand (the effect if people stayed on treatment), mean weight loss reached 16.6% versus 3.2% on placebo. Up to 85.1% lost at least 5% of body weight, versus 38.8% on placebo.

Absolute loss was reported as up to 39.2 lb (17.8 kg), and waist circumference also fell versus placebo.

Read the estimand. Efficacy numbers assume people remained on drug. The treatment-regimen estimand, which keeps dropouts in the average, is the figure regulators watch, and Boehringer did not publish those percentages in the topline release.

Gastrointestinal events were again the main tolerability story, with discontinuations clustering during dose escalation. No new safety signal outside the GLP-1 class was claimed. Survodutide has not been approved, and catalog “survo” vials are not the GMP product in SYNCHRONIZE.

Licensed incretin products, when they exist, show up in pharmacy and telehealth channels, which is what the provider directory and price index compare. Handling education lives on the reconstitution guide and the calculator. Clinician voices on metabolic peptides are collected on experts.

Lean-mass questions after large GLP-1 losses are covered in GLP-1 peptides and muscle. Boehringer said most SYNCHRONIZE-1 loss was fat. That claim is company data.

Bottom line: Survodutide is a real glucagon/GLP-1 dual agonist peptide with phase 2 obesity and MASH biopsy data, plus a company phase 3 obesity topline. It is a different receptor story from semaglutide. It is not an approved drug, and a research vial is not the trial product.
