Setmelanotide is a cyclic eight-amino-acid analog of alpha-melanocyte-stimulating hormone (α-MSH) that turns on the melanocortin-4 receptor (MC4R), the hypothalamic switch that says stop eating. The FDA-approved product is Imcivree. It is a peptide for rare, pathway-broken obesity, not a weekly incretin for typical weight loss.
TL;DR
- MC4R sits downstream of leptin and POMC neurons. When that path is broken, hunger does not shut off.
- Setmelanotide is a cyclic octapeptide that activates MC4R directly, bypassing missing leptin or POMC signals.
- POMC/PCSK1 trials: 8 of 10 people lost at least 10% of body weight in about a year. LEPR: 5 of 11 did.
- TRANSCEND (2026): in acquired hypothalamic obesity, mean BMI fell 16.5% on drug and rose 3.3% on placebo at 52 weeks.
- The label excludes general (polygenic) obesity. Hyperpigmentation is common because the peptide also hits MC1R.

What is setmelanotide, and how is it different from a GLP-1?
Most weight-loss peptide conversations now start at GLP-1 or GIP receptors. Setmelanotide does not. It is an MC4R agonist, a synthetic stand-in for α-MSH, the POMC-derived peptide that normally says “enough.”
It is cyclic, eight residues, with a disulfide that locks the His-Phe-Arg-Trp melanocortin core. Cryo-EM of the MC4R-Gs complex puts that core in the same pocket as NDP-α-MSH, the linear analog sold as afamelanotide (Heyder et al., 2021).
Selectivity is relative. Setmelanotide prefers MC4R but still activates melanocortin-1 receptor (MC1R) on melanocytes, which is why skin darkens in most treated patients. That is the same receptor family behind KPV and bremelanotide (PT-141).
Semaglutide versus tirzepatide is a different article. For the chemistry primer, see Peptides 101.
How does the leptin-melanocortin pathway control hunger?
Fat tissue releases leptin, and leptin-receptor (LEPR) neurons then tell POMC neurons to make α-MSH, which hits MC4R so appetite falls and energy use rises. Agouti-related peptide does the opposite. Break any step and the brake fails:
- POMC deficiency: little or no α-MSH is made.
- PCSK1 deficiency: POMC is not processed into α-MSH.
- LEPR deficiency: the leptin signal never arrives.
- Bardet-Biedl syndrome (BBS): cilia trafficking impairs leptin-MC4R signaling.
- Acquired hypothalamic obesity: a tumor, surgery, radiation, trauma, or inflammation damages the same circuitry.
Setmelanotide is designed to skip the broken steps and push MC4R itself. That is why the label is narrow. Ordinary polygenic obesity is the wrong receptor story.
What did the genetic-obesity trials actually measure?
The first approvals were not lifestyle-obesity trials. They were small studies in people obese since early childhood because a gene in this path did not work.
| Study | Who was enrolled | Design | Main finding |
|---|---|---|---|
| Clement 2020, POMC/PCSK1 | 10 people, age 6 and older | Open-label, about 1 year | 8 of 10 (80%) lost at least 10% body weight |
| Clement 2020, LEPR | 11 people, age 6 and older | Open-label, about 1 year | 5 of 11 (45%) lost at least 10% body weight |
| Haqq 2022, BBS | Patients 6 and older with BBS (Alstrom inconclusive) | Phase 3, then open-label | 32.3% of those 12 and older lost at least 10% at 52 weeks |
| TRANSCEND 2026, acquired HO | 120 people, ages 4 to 66 | Randomized 2:1, 52 weeks | Mean BMI -16.5% vs +3.3% placebo |
FDA approved Imcivree on November 27, 2020, for genetically confirmed POMC, PCSK1, and LEPR deficiency in patients 6 and older. The agency was explicit: it is not approved for general obesity (FDA, 2020).

What did TRANSCEND show in acquired hypothalamic obesity?
Acquired hypothalamic obesity follows injury to the hypothalamus, most often a craniopharyngioma or its treatment. Hunger becomes relentless and weight climbs fast. GLP-1 drugs do not repair that circuit.
TRANSCEND tested whether an MC4R agonist could still work after structural damage. Investigators randomized 120 patients, ages 4 to 66, 2:1 to daily subcutaneous setmelanotide or placebo for 52 weeks. Average age was 19.9, and 78% had a craniopharyngioma as the cause (Miller, Roth et al., 2026).
BMI fell 16.5% on setmelanotide and rose 3.3% on placebo (P<0.001), about 36.4 lb lost versus 5.3 lb gained. Weekly maximum hunger scores fell 2.73 points versus 1.45 (P=0.009). Eighty percent on drug reached at least a 5% BMI cut, versus 10% on placebo.
On March 19, 2026, FDA expanded Imcivree to patients 4 years and older with acquired hypothalamic obesity, the first labeled therapy for it. Rhythm reported an 18.4% placebo-adjusted BMI difference in 142 patients and estimates about 10,000 U.S. cases (Rhythm, 2026).
What are the catch and the catalog problem?
Hyperpigmentation is the mechanism on skin. In TRANSCEND, darkening occurred in 56% on drug versus 8% on placebo, and nausea, vomiting, headache, and new or darkening moles (17% vs 5%) were also more common. The label warns about sexual-arousal changes, depression and suicidal ideation, hypersensitivity, and, in acquired HO, adrenal crises and sodium swings.
Limitations of use are the point. Setmelanotide is not indicated for benign POMC/PCSK1/LEPR variants, other unlabeled genetic syndromes, or general polygenic obesity. Forum vials labeled setmelanotide or melanotan are not Imcivree. Identity is a COA question.
Licensed product lives in pharmacy and telehealth channels, which is what the provider directory and price index compare. Handling education lives on the reconstitution guide and the calculator. Clinician voices on this pathway are collected on experts.
Bottom line: Setmelanotide is a real MC4R agonist peptide with genetic-obesity and hypothalamic-obesity trial data behind it. It is proof that the melanocortin path matters. It is not a GLP-1, and it is not a general-obesity research stack.
Educational only. Not medical advice. Not a recommendation to use Imcivree, setmelanotide, or any research-grade melanocortin peptide.
