FOXO4-DRI is a synthetic D-retro-inverso peptide fused to an HIV-TAT uptake sequence. Baar, de Keizer, and colleagues designed it to compete with forkhead box O4 (FOXO4) for p53, so senescent cells holding p53 in nuclear repair foci would dump that p53 and die. It is not a natural hormone, and there is no FDA-approved FOXO4-DRI product.
TL;DR
- FOXO4-DRI is a designed mimic, not a digested transcription factor. It copies a FOXO4 stretch that binds p53, written in D-amino acids in reverse order, then tagged with TAT so it can enter cells (Baar et al., Cell 2017).
- Senescent IMR90 fibroblasts raise FOXO4 and keep p53 in PML bodies next to DNA-SCARS. Breaking that pair excluded Ser15-phosphorylated p53 from the nucleus and triggered caspase-dependent apoptosis in senescent cells, not in matched proliferating controls.
- Mouse work used pulsed peptide after damage or in aged animals. Sequential FOXO4-DRI after doxorubicin lowered liver FOXO4 foci, IL-6, plasma AST, and urea. Fast-aging and naturally aged mice showed better fur, responsiveness, and kidney filtration markers.
- A 2025 NMR paper mapped the contact. FOXO4’s forkhead domain and FOXO4-DRI both bind the disordered p53 transactivation domain 2 (TAD2). Phosphorylation at Ser46 and Thr55 tightens that grip (Bourgeois et al., 2025).
- A catalog vial is not that lot. The 2017 peptide is patent-restricted research material. No completed human trial of FOXO4-DRI has been published.
Educational only. Not medical advice, not a senolytic protocol, and not a reason to treat a research vial as a longevity drug.
What is FOXO4-DRI, chemically?
It is a cell-penetrating D-peptide, not FOXO4 itself. D-retro-inverso chemistry flips the sequence and uses D-amino acids so proteases cannot chew it the usual way. Baar’s group picked a FOXO4 region conserved in humans and mice but different from FOXO1 and FOXO3, then fused HIV-TAT for uptake.
TAT staining showed peptide inside senescent IMR90 within 2 to 4 hours, still detectable at 72 hours. NMR competition showed FOXO4-DRI pulling p53 off the FOXO4 forkhead domain with higher affinity than the natural partner (Baar et al., 2017).
Solid-phase synthesis can make a TAT fusion. That chemistry is covered in how peptides are made and Peptides 101. Reconstitution, if a lab works with powder, is a chemistry step in the reconstitution guide and the calculator, not a dosing chart.
How does FOXO4 keep senescent cells alive?
Senescent IMR90 looked apoptosis-primed, then refused to finish the job. After 10 Gy ionizing radiation, PUMA and BIM rose and BCL-2 fell. FOXO4 mRNA and protein climbed. FOXO1 and FOXO3 barely moved.
Knocking FOXO4 down before senescence released cytochrome c and cleaved caspase-3. Knocking it down after senescence cut viability in senescent cultures, not in controls.
The geography matters. FOXO4 accumulated in PML nuclear bodies next to 53BP1-containing DNA-SCARS, beside ATM-phosphorylated p53 (Ser15). In that pocket p53 can hold a p21Cip1 arrest. It is less free to leave for mitochondria and kill the cell.
FOXO4-DRI reduced those FOXO4, PML, and 53BP1 clusters, dropped nuclear p21, and drove Ser15-p53 out of the nucleus. Pan-caspase blockade and p53 shRNA each blunted the kill. The claim is selective, p53-dependent apoptosis in senescent fibroblasts, not a general cytotoxin.
Senescence is also a brake on damaged division. Clearing it in mice is not a human anti-aging approval. Related longevity peptides on this site start with humanin and SS-31 / elamipretide.
What did the 2017 mouse studies actually show?
The in vivo arms are short, pulsed, and mouse-only. Read them as tissue-homeostasis experiments, not lifespan trials.
| Study | Who and how | What the paper reported |
|---|---|---|
| Baar 2017, doxorubicin | Mice after 2 intraperitoneal doxorubicin doses (10 mg/kg), then 3 intravenous FOXO4-DRI doses at 5 mg/kg, every other day | Body weight, liver FOXO4 foci, liver IL-6, plasma AST, and urea moved back toward control (PMID 28340339) |
| Baar 2017, XpdTTD/TTD | Fast-aging trichothiodystrophy model, high p16 reporter by 26 weeks | Lower p16 luminescence, better fur and running-wheel activity, lower plasma urea at 30 days |
| Baar 2017, naturally aged | p16 reporter mice about 110 to 130 weeks | Platelets did not drop. Fur, kidney lamin-B1, IL-6, urea, and creatinine improved. Ganciclovir clearance of p16-positive cells moved the same kidney markers |
| Huang 2021, chondrocytes | Human knee chondrocytes at PDL9 versus PDL3, 25 uM for 5 days | More than half of PDL9 cells gone, PDL3 spared, SA-beta-gal in PDL9 below 5%. Pellet culture did not make more cartilage (PMID 33996787) |
| Zhang 2020, Leydig cells | Naturally aged mice | Senescent Leydig-cell burden fell. Serum testosterone rose versus old controls (PMID 31959736) |
The doxorubicin arm is sequential, not concurrent. Giving FOXO4-DRI around the time of DNA damage did not sensitize healthy IMR90. Giving it after senescence had already formed did.
The aging arms restored function that had already slipped. Urea and creatinine are filtration markers, fur and running wheels are frailty readouts, and none of those papers measured median lifespan on FOXO4-DRI.
Huang’s follow-up is the same kind of honesty. Selective kill of senescent chondrocytes in a dish is real. Better cartilage in a pellet is not automatic.
Did later work change the mechanism?
Bourgeois, Spreitzer, de Keizer, Madl, and colleagues mapped the contact in 2025. FOXO4’s forkhead domain binds p53 TAD2 (about residues 45 to 57). FOXO4-DRI binds the same stretch and competes with the natural pair (Nat Commun 2025).
Both partners start disordered and fold on contact. Phosphorylation of TAD2 at Ser46 and Thr55 raised affinity. Krimpenfort and Berns, commenting in the same 2017 Cell issue, also flagged that senescence is a tumor-suppressive state (PMID 28340347).
How is a research vial different from the 2017 peptide?
Endogenous FOXO4 is a transcription factor you already make. FOXO4-DRI is a laboratory mimic of one interaction surface. Identity, salt, and endotoxin are COA questions, not label promises.
There is no FDA-approved FOXO4-DRI drug. Licensed peptide products live in pharmacy and telehealth channels that the provider directory and price index compare. Catalog FOXO4-DRI is a research chemical, and the 2017 authors noted patent and MTA limits on sharing the exact peptide.
For another longevity-peptide claim, see epitalon.
Bottom line: FOXO4-DRI is a real TAT-tagged D-peptide that, in the 2017 Cell study, selectively killed senescent fibroblasts and improved chemotoxicity and kidney markers in mice. A catalog vial is not an approved senolytic, and no published human trial has tested it.
Educational only. Not medical advice. Not a recommendation to use FOXO4-DRI or any research-grade senolytic peptide.
